What We Collect

Personal Financial Statement (signed and dated within 90 days of submission)

Past three years of filed tax returns (signed and filed versions);
copy of extension for current year if appropriate; K-1’s for all submitted returns.

Driver’s License

Copies of Statements that support liquidity positions on the PFS – bank statements, brokerage statements.
